LICENSED CLNICAL SOCIAL WORKER

JOB SUMMARY:

Is responsible for working with clients of all ages cope with and solve everyday problems. This position is the link between the community and providers, and works collaboratively with all clinical departments to ensure best work environment for the clients.

DUTIES AND RESPONSIBILITIES:

  • Observe client behavior, assess needs and create treatment strategies.

  • Diagnose psychological, behavioral and emotional disorders.

  • Help clients deal with issues such as addiction, abuse, or mental illness.

  • Provide social work interventions to remove psychosocial barriers to health and wellness.

  • Consult with doctors, therapists and medical professionals

  • Provide close monitoring of patients through follow up contacts and coordinate services with other providers as necessary.

  • Provide crisis intervention; assess and coordinate protective service issues.

  • Document interventions in patients’ medical records.

  • Maintain knowledge of resources available in the area community.

  • Administer social service programs

  • Instruct clients’ families during treatment

  • Advocate for and develop plans to improve clients’ well-being

  • Research and refer clients to community resources.

  • Provide and maintain educational materials.

  • Manage a specific case load of patients referred by clinic staff.

  • Maintain current in the field by attending seminars, continuing education courses, workshops, etc.

Additional Duties

  • Develop clinical programs and materials.

  • Provide staff education.

  • Interact with community agencies involved in health and social services, represent WCHC at meetings and events in the community.

  • Promote positive working relationships with community agencies and with other departments.

  • And all other duties as assigned.

KNOWLEDGE, SKILLS AND ABILITY:

  • Ability to develop treatment plans.

  • Ability to both lead and follow in a team environment.

  • Ability to recognize and appropriately act on opportunities.

  • Ability to connect clients to community resources.

  • Excellent documentation skills.

  • A passion for working with at-risk, culturally diverse populations.

  • Ability to communicate effectively both written and oral.

  • Ability to find and use resources.

EDUCATION AND EXPERIENCE

  • Master of Social Work (MSW) required.

  • State licensing required.

  • Advanced Alcohol and Drug Abuse Counselor (CAADC) or related certification required

  • At least 2 years of post-degree clinical experience.

  • Experience with mental health and substance abuse preferred.

  • Experience in community social services and/or health care.

  • Bilingual preferred
