ABOUT THE ORGANIZATION The mission of the National Kidney Foundation of Michigan (NKFM) is to prevent kidney disease and
improve the quality of life for those living with it. All of our programs are focused on this mission. The
two leading causes of chronic kidney disease (CKD), diabetes and hypertension, account for 70% of all
Michigan kidney failure cases. If signs and symptoms remain undiagnosed/untreated, an estimated twothirds
of individuals with diabetes and hypertension will develop CKD, which can progress to kidney
failure.

Overweight/obesity and lack of physical activity are two of the leading causes of diabetes and hypertension
among children and adults. Kidney failure can be prevented or delayed, in large part, through early
identification, medication adherence, and lifestyle changes such as increased physical activity and healthy
eating. Not everyone goes to their doctor on a regular basis. Unsurprisingly, silent diseases are often
undiagnosed for many years. Community- based programs and health care systems change programs,
focusing on those at risk, are essential to prevention of CKD.

The NKFM is the largest affiliate in the country and delivers more programs and services to more people
with chronic kidney disease than any other NKF affiliate or organization in the country. Last year, the
programs described below directly served more than 80,000 individuals. The strength of the NKFM is
a diverse funding base which includes special events, federal, state and private dollars, and individual
contributions. The NKFM has received numerous national awards for its outstanding programs and
services and has received the coveted four star rating from Charity Navigator for the past 9 years, an
accomplishment that only 1% of charities nationally can tout.  
DESCRIPTION

Under general supervision, a Family Support Coordinator will enhance the quality of participant management and maximize satisfaction. The Family Support Coordinator conducts comprehensive and coordinated case management for families with young children (0-5). This includes developing and maintaining partnerships with key organizations, providing leadership, in conjunction with the supervisor, in the implementation of programs that are a focus of grants/contracts and of the NKFM strategic plan specific to prevention of CKD and its precursor's; obesity, diabetes and hypertension. The coordinator will work with community partners to recruit and enroll new participants for NKFM programs. The specialist will be accountable for the recruitment of and ongoing support and engagement of families with young children to engage in a family support intervention. The Family Support Coordinator duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the families health and human service's needs through the use of motivational interviewing while using a standardize assessment tool. Will receive administrative and other assignments from the supervisor from time to time.

Major Job Duties and Responsibilities

  • Oversight of programs as assigned. Develop and maintain a positive working relationship with the program personnel and community partners.
  • Recruitment of program participants for community based health education and family support programs.
  • Conduct family assessment of needs through motivational interviewing and provide ongoing support as needed.
  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
  • Handle case assignments, draft service plans, review case progress and determine case closure
  • Help families achieve wellness and self-sufficiency
  • Facilitate multiple care aspects (case coordination, information sharing, etc)
  • Provide referrals and resources to families as appropriate
  • Record cases information, complete accurately all necessary forms and produce reports
  • Ensure data collection and administrative forms are collected, accurately completed, and entered in a timely manner.
  • Adhere to professional standards as outlined by protocols, rules and regulations
  • Integrate educational efforts, focusing on increasing awareness regarding the prevention of kidney disease and promoting programs that are available that improve the quality of life for people on dialysis in low income areas.
  • Follow all guidelines of the program curriculum or expectations with a high level of detail and accuracy.
  • Insure requirements of grants/contracts.
  • Develop community partnerships for recruitment and engagement.
  • As assigned, provide data entry, answer phones, complete daily reports, recruitment/marketing material development, etc.
  • Ability to take on additional responsibilities and implement multiple programs.
  • Supports others with the team problem solving process. Prompts others to contribute knowledge and information to assist the team. Encourages the team to remain focused and on track and resolves any interpersonal issues arising that may jeopardize team success.

POSITION REQUIREMENTS

Education and Experience

  • Bachelor’s Degree in public health, social work or in a human services related.
  • Basic knowledge of MS Office & MS Outlook computer software.
  • Experience in Motivational Interviewing, and/or experience with coaching, behavior change programming, and working with families and individuals in a therapy or social work setting.
  • Demonstrates basic knowledge of health education.
  • Experience working with families and conducting assessment of needs and/or case management preferred.

Working Conditions

  • Primarily field work with office work required. Evening and weekend work occasionally required to successfully implement responsibilities. In state and possibly some out of state travel may occur, including occasional over-night stays.
  • Physical requirements; light physical effort equal to lifting or moving of lightweight materials. Regularly required to sit or stand, bend and reach.
  • Must have access to reliable transportation, a valid driver’s license and up to date auto insurance.
