Summary:

The Care Manager is primarily responsible for overseeing the care of the clients that they manage on their caseload, communicating effectively with the professional team, clients and their families.

Job Responsibilities:

  • Meet with clients once weekly unless otherwise directed by the insurance company or SAI management.
  • Communicate with Field Manager weekly to report any significant issues or problems.
  • Attend and participate in client team meetings as scheduled by providers and/or case managers. If there is not a team meeting scheduled it is the Care Managers responsibility to coordinate the scheduling of a team meeting if deemed necessary.
  • Attend medical appointments and other meetings deemed necessary as scheduled to advocate on behalf of the client.
  • Communicate on a regular basis with the client’s team members such as residential facilities, psychiatrists, physiatrists, etc. in addition to providing updates to the Co-Guardian if there is one assigned to the case.
  • Communicate effectively and respectfully with internal SAI co-workers.
  • Periodically, Care Manager will be called upon to assist clients in enhancing community integration skills by taking them out into the community.
  • Care Managers are encouraged to ask any questions and bring concerns to the Field Manager.
  • Care Managers are expected to attend monthly staff meetings. If the Care Manager can’t attend, they are to notify the Field Manager before the meeting and arrange a meeting to receive updates.
  • The Care Manager is expected to be familiar with SAI policies and procedures as presented in the employee policies and procedures manual.
  • The Care Manager is expected to complete client Advocate documentation weekly per policy.
  • Submit any client reimbursement to the SAI office weekly.
  • Submit mileage reimbursements to the SAI office monthly.
  • The Care Manager Monthly report and Quarterly Report is to be completed by the 7th working day of the following month per policy.

Qualifications:

  • Bachelor's degree from an accredited university in social work, or other closely related medical or human services field of study.
  • Master's degree in Social Work preferred.
  • Working knowledge of traumatic brain and spinal cord injury and its physical, cognitive, functional and behavioral implications preferred.
  • A high level of executive functioning skills including planning, organizing, implementing/executing, coordinating, managing time and resources.
  • Excellent verbal and written communication skills
