Job Description Summary:

The STIRR Housing Resource Community Coordinator is a key member of the Community Housing Network’s Community Programs team. This position is a full time salary position and will report directly to the Community Programs Manager. The STIRR Housing Resource Community Coordinator will act as an enhanced housing and community resource navigator for those eligible program participants that are referred directly from the OCHN STIRR program, identified as literally homeless upon discharge from Oakland County Jail (OCJ) from the STIRR program coordinator located at Oakland County Jail, or through assignment to HOPE Adult Shelter upon discharge, or identified as CAT 1 within a 30 day discharge from jail. The STIRR Housing Resource Community Coordinator will assist those with OCHN services who are referred by providing screening and linkage to available housing programs and resources based on their specific circumstances. This will include short term, medium term as well as long term housing planning. The STIRR Housing Resource Community Coordinator will remain the housing case manager for the program participant throughout their enrollment in this program, as they navigate through financial assistance resources, with the intent to provide services to increase self-sufficiency or when the program participant has identified a long term subsidy. This position will also take an active role in responsibilities related to the coordinated system of delivery and working in collaboration with community partners and members of the Continuum of Care (CoC) to ensure quality of service delivery and work towards goals of the individual program participants as well as performance outcomes for the community to include a special focus on assisting those who are returning to the community.


Essential Functions:

  • Meet with the identified potential program participant in the field at OCJ and evening shift at HOPE Shelter to complete a full assessment to determine eligibility for available programs and resources. This will include review of available financial assistance. Perform housing assessment of need for STIRR referrals by STIRR Program Staff and/or HOPE Adult Shelter. This will include a full screening that is utilized for Coordinated Assessment as well as a VI SPDAT.
  • Perform “reverse referrals” regarding OCJ/OCHN individuals when presenting CAT 1
  • Housing search and placement activities to assist program participants in locating, obtaining, and retaining housing. Develop and maintain positive working relationships with area landlords and public housing authorities (PHA’s), maintaining a list of available units and condition of local housing stock that includes rental market, state and federal housing programs in the community, working to expand resources for those who are returning community members.
  • Assist STIRR program participants with collecting documentation necessary to apply for assistance in short term financial assistance programs (i.e. RRH) as well as longer term housing options (i.e. Housing Choice Vouchers – Homeless Preference, Permanent Supportive Housing, Supportive Housing, Subsidized Housing).
  • Conduct home visits with program participants to review housing goals and continued eligibility. This will include review of documentation, recertification, full SPDATs, budget review and linking with additional community resources as prescribed by agency and funding requirements. Provide housing case management directly to the STIRR program participant while receiving short term financial assistance to provide more intensive housing case management with a strength based approach, developing an individualized housing plan in collaboration with the individual/household receiving services. Work on goals related to self-sufficiency. Housing Case Management will be provided within the identified housing unit, or within the community per the program participant’s request, on at least a monthly basis with additional meetings as needed/required.
  • Provide linking and referral to other necessary services to include but not limited to referrals to employment, health care, substance abuse, and related community services. Document all contacts and produce appropriate reports for grantor, including time activity sheets, and tracker of all STIRR program participants. Facilitate a bi weekly web call with colloaborating agencies for STIRR grant. Enter all program participants as well as screened/assessed households within the HMIS system in a timely manner per grant and agency requirements
  • Explore and identify additional community resources to benefit program participants in linking with available programs and services. Maintain system to triage and prioritize program participants to ensure that proper services are provided while minimizing service duplication. Interface with other departments internally (such as PSH, PATH, HRC) to coordinate and streamline processes.
  • Assuring the rights of program participants are protected. Maintain documentation in both an electronic and paper record while adhering to all confidentiality and privacy standards.
  • Research and disseminate information regarding access and coordination of mainstream, housing, mental health, and other community based resources with mental health resources.
  • Participate in community based outreach activities as requested and appropriate such as but not limited to PIT Count, Community Resource Day, in-reach to local shelters and partnering agencies and CoC events. Some activities will include evening and weekend hours.
  • Continue to refine skills and stay abreast related to best practices and program changes through additional trainings and community events as requested by the supervisor.
  • Other duties and projects as assigned

Qualifications/Requirements:

  • Bachelor’s degree in social work or human services related field; or five years of human service related experience and/or training, or equivalent combination of education and services.
  • Working knowledge of the CES and other community resources and services related to housing and supports is essential.
  • Must possess proficient computer skills, and be well organized, proactive and self-directed. Must also be attentive to details and to multi task in a fast paced environment.
  • Reliable transportation, possession of a valid driver’s license, and maintenance of automobile insurance coverage that meets organizations coverage limit requirements. (Bodily Injury $100,000 each person and $300,000 each occurrence and Property Damage of $100,00 each occurrence). CHN reserves that right to require a motor vehicle record check with respect to any employee where driving is an essential function of the position

Physical Demands:

While performing the functions of this job, the position is both sedentary and at times requires the ability to be active including standing, walking in the outside elements, in and out of vehicle, bending, lifting and/or arranging files and office products and supplies and may require employee to periodically stand on a short step stool to access files.
