Under the general supervision of Residenal Service Director, the Residenal Care Specialist (RCS) is responsible for the overall accountability for the idenficaon and supports to members appropriate for residenal care management services. RCS completes a comprehensive inial assessment, reassesses, and gathers pernent informaon about the member’s needs by interviewing the member, appropriate family members, physicians and other members of the Muldisciplinary Care Team to develop, monitor and evaluate the member’s residenal and/or supporve service plan and updates as needed. RCS will acvely guide and facilitate transions of care, supports the development of, monitoring and management the care coordinaon acvies related to the Residenal Service and/or Supporve Service programs (SIL and CLS).

Essenal Job Functions:

The Residenal Care Specialist provides specialized behavioral assessment, intervenon services and program supervision individual seeking and receiving residenal services. They are responsible for the review and authorizaon of individualized residenal service programs for each consumer. They provide clinical supervision & support to the Provider Network Residenal Care Coordinators.

Clinical Responsibilies:

  • Supervises individualized the review and authorizaon of residenal services and programs for individuals diagnosed with a serious mental illness, serious emoonal disturbance, intellectual developmental disability and co-occurring substance use disorder.
  • Assures consumers receive intervenons to meet the full extent of the services authorized;
  • Coaches clinical teams on the procedural implementaon of the individual plan of service, treatment plans via team meengs, telephonic and live/in supervision.
  • Reviews data and reports submied by Residenal Care Coordinators, direct care team, and recommends changes to the program, procedures, and data collecon methods to assure programs reflect each individual’s development and progress towards idenfied goals.
  • Observes and assesses consumer skills and progress to inform recommendaons for ongoing intervenon and behavioral programming.
  • Collaborates with addional service providers on the consumer’s support team to ensure comprehensive service delivery.
  • Addresses program delivery with families; including caregiver coaching, challenges with implementaon of program and placement, caregiver parcipaon, and work environment as necessary.
  • Reviews progress reports and treatments plans, ensuring that they meet the requirements of regulatory agencies and funding sources and are clinically accurate and appropriate.

Administrative Responsibilies:

  • Maintain and ensure accurate documentaon of billable tasks that meets the requirements of regulatory agencies and funding sources, and is HIPAA compliant.
  • Track direct and indirect consumer/member sessions to ensure that services are provided to the full extent of the authorizaon for each consumer/member.
  • May parcipate and/or support the recruitment, interviewing, selecon, training and onboarding of residenal provider staff.
  • Coordinate documentaon to ensure reauthorizaon and connuaon of services when necessary.

Supervisory Responsibilies:

  • Provide regular performance feedback to Residenal Service Director related to their strengths and areas for improvement,
  • Support to Residenal Service Director in development, implementaon, and oversight of residenal service management and provider service and treatment plans
  • Ensure DWMHA policy and procedure is communicated to and understood by residenal provider staff.
  • Attends staff meengs, trainings, and other meengs as requested.
  • Responsible to complete addional job dues as assigned by to Residenal Service Director.

General Responsibilies:

  • Provide guidance and oversight of process for all housing and in home supporve service referrals and request.
  • Advocate and liaison for MI, IDD, SUD members with Affiliated Providers
  • Provide guidance and oversight to implementaon of Care Management protocols related to behavioral health concerns insuring the integraon of the bio-psychosocial and other assessments, i.e., LOCUS, SIS and ASAM
  • Provide guidance and oversight to residenal team meengs, pre-release meengs, placement review commiee meengs and discharge planning meengs.
  • Engage and verify data collecon for the purpose of assessing, care coordinaon and authorizaons of care
  • Provide technical assistance, guidance and oversight for Affiliated providers on behavioral care coordinaon pracces.
  • Provides explanaon of behavioral health service and benefit
  • Required traveling to and accessing members' in hospitals, homes and community.
  • Required 50% of me will be spent traveling.
  • Preferred community based care experience.

Qualifications (Skills & Abilies):

  • Knowledgeable of evidence-based methodologies found to benefit individuals in need of residenal care and services
  • Extensive training in the field of assessment, treatment planning and placement as well as knowledgeable of other community resources and agencies that serve people diagnosed with a serious mental illness, serious emoonal disturbance, intellectual developmental disability and co-occurring substance use disorder.
  • Sensive to working with an ethnically, linguiscally, culturally, and economically diverse populaon.
  • Able to communicate effecvely verbally and in wring; American Sign Language or bilingual ability preferred.
  • Able to consistently demonstrate good judgment and decision-making skills.
  • Able to exercise confidenality and discreon pertaining to the work environment.
  • Able to appropriately interpret and implement policies, procedures, and regulaons.
  • Knowledgeable and skilled in computer/word processing soware and excel.
  • Able to travel to mulple work sites; reliable transportaon needed (A valid State of Michigan Driver’s License, current auto insurance idenficaon card, and maintain acceptable driving record per MDHHS standards is required).

Person interested in being considered must possess the following:

1. Minimum Master’s Degree in social work or psychology or counseling or Associated Degree in Nursing (Only);

2. Licensed by the State of Michigan as a fully Licensed Psychologist (LP), or Limited Licensed Psychologist (LLP) or Licensed Masters Social Worker (LMSW) or Licensed Professional Counselor (LPC) or Licensed Register Nurse (RN or Limited Pracce Nurse (LPN)

Education:

  • Associates degree in Nursing, Bachelor’s preferred
  • Degree in of the following: Counseling, Social Work, Psychology, Master’s required

Experience:

  • Minimum of five years’ experience working in mental health services, experience working with a managed care organizaon, desired.
  • Experience must have been in the following:
  • Experience in the provision of Behavioral Health Benefit Services
  • Assessment and Utilizaon Management Review
  • Experience working with adults who are seriously mentally ill, or person with a developmental disability or children who have serious emoonal disturbances or elderly persons with co-occurring mental illness and substance use disorders.
  • Experience with co-morbid condions. Cultural competencies training required.

Certification/Licensure:

  • The RCS must be an acve social work or psychology or counselor or nurse license in the State of Michigan:
  • Licensed Registered Nurse (RN);
  • Licensed Professional Counselor (LPC or LLPC);
  • Limited Licensed Psychologist, (MA or MS, LLP);
  • Licensed Bachelor’s prepared social worker (BSW); must meet min. requirement of 5 years’ clinical experience;
  • Limited License Master’s prepared social worker (LLMSW); or
  • Licensed Master’s prepared social worker (MSW).
