Description: Case Manager I: $41,806 Starting; Case Manager II: $48,077 Starting; Case Manager III: $50,481 Starting Under the supervision of a Mental Health Program Director and Program Coordinator, full-time case manager will provide assessment, case management, crisis intervention and integrated care coordination services to people with serious mental illness and possibly co-occurring substance use disorders in community and office-based settings. Case manager must function well as part of a cohesive team and have good knowledge of community resources.

Duties: Regular full-time case manager for adults with serious mental illness. Position includes, but is not limited to: assessment, person-centered planning, referrals, assisting with benefit procurement, crisis intervention, advocacy, skills training, support with taking medications, care coordination with healthcare providers and connecting people with community resources.

Qualifications: Case Manager I: Bachelor's degree in social work or equivalent with LBSW or LLBSW required. If licensed at LLBSW level, individual must obtain full LBSW within two years of hire. Case Manager Level II: Master's degree in social work or equivalent with LLMSW or equivalent required. If licensed at LLMSW level, individual must obtain full LMSW within two years of hire. Case Manager Level III: Master's degree in social work or equivalent with LMSW, LPC or LLP. All case manager levels: Strong teaming skills. At least one year experience working with people with mental illness, preferably in a CMH setting. Experience with electronic records preferred. Valid MI driver’s license required. Some evening hours may be required. EXPERIENCE: Prior treatment experience working with people with serious mental illness required. Prior experience working in a community mental health system preferred. Experience in working with electronic medical records preferred. Ability to work as part of a team of professionals in a public mental health setting required. Strong assessment skills preferred.
