Posting: The Youth Connection - Youth Services Case Manager Child Welfare

The Youth Case Manager will provide Youth in Transition (YIT) closed case services to assist clients in achieving the goal of becoming self-sufficient. These services include, but are not limited to housing assistance, employment support, and case management. Case Manager will assist with intake and enrollment, assessing client needs, developing, implementing, service/action plans and progress, facilitating data-driven, creative, and strategic approaches to servicing clients, and provide ongoing case management. Also, works in coordination with other TYCCA programs and closely with MDHHS staff to target eligible participants with unmet needs to provide counseling, referrals, and other supportive services to support growth and success. Must possess excellent interpersonal and communication skills with a demonstrated ability to connect with youth, young adults, and parents where appropriate.
RESPONSIBILITIES AND DUTIES:
- Work closely with clients to develop and implement individual, youth-driven action plans to identify and meet clear, achievable, realistic action steps and life goals and monitor progress.
- Provide intensive, individualized case management services, identify and secure community resources, provide and/or refer to counseling or mental health services, give guidance and direction on how to navigate adulthood and overcome barriers, and appropriate decision making.
- Assist clients with determining eligibility, signing up, and accessing community and social service programs as needed and provide transportation as necessary for clients (either directly or indirectly) to ensure clients are able to take advantage of needed services and all required program activities.
- Assess client employability skills and goals and provide individualized support as needed including job placement assistance, assistance in addressing barriers to employment, and work readiness training.
- Develop and facilitate life skills and other trainings and workshops to develop the skills necessary to achieve economic independence and successful home management; also provide crisis intervention and assist youth and families in understanding the implications and complexities of their individual situations and lifestyles.
- Maintain a minimum of monthly contact with all clients making additional contact as needed, complete detailed, monthly case notes and program documentation for all clients and services provided to ensure appropriate follow-up and reporting; provide information for and assist with program reports as needed.
- Assist as needed with case work flow, receipt of vital documents and all paperwork per the contract terms (i.e. Updated Service Plan, Child Assessment of Needs and Strengths, Casey Life Skills Assessment, Individualized Action Plan, etc.).
- Ensure client services are facilitated and coordinated with other federal, state, and community programs including, but not limited to, homeless/housing programs, educational and school-to-work programs, employment programs, sexual health, pregnancy prevention, food programs, health care services, and help clients access YIT services, submit YIT requests, and ensure services provided demonstrate measurable progress and attainment of expected performance outcomes.
- Assist with promoting program and recruiting participants, referrals, intake and enrollment processes
- Assist with client follow-up to assist clients as needed in maintaining self-sufficiency after receiving services to measure outcomes.
- Perform a variety of tasks as needed to help improve the lives of all clients and participants.
CONDITIONS OF EMPLOYMENT
- Ability to work flexible hours including late evenings, weekends, and on-call.
- Must have reliable transportation and willing to travel within the cities TYC serves.
- Must provide State of Michigan I-CHAT background clearance, DHHS Central Registry clearance, and local police clearance. Federal clearances may also be required and State of Michigan Sex Offender Registry check.
- Must submit a writing sample to demonstrate effective communication skills.
REQUIRED JOB KNOWLEDGE, SKILL AND ABILITY:
- Must possess an intimate knowledge of child welfare, social services system, rules and regulations regarding the management of youth cases, specifically youth who have aged out of foster care.
- Must be familiar with standard concepts, practices, and procedures within the field of social work, including effective case noting and documentation for reporting purposes.
- Must have a knowledge of administrative and technology procedures and systems such as word processing, managing files and records, designing forms, and other program procedures and terminology.
- Must be able to manage multiple tasks simultaneously, and follow through on issues in a timely manner and utilize time management skills to organize and prioritize work to meet program deadlines and desired outcomes.
- Must possess excellent verbal communication and writing skills with knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar.
- Must have computer skills to prepare documents and presentations, maintain various databases, spreadsheets, to track program data/outcomes, and search for information on the internet.
- Must possess strong knowledge of MS Office, including Word, Excel, Access, PowerPoint and Outlook.
- Must be highly organized and have great attention to detail
Bachelor’s Degree in the human services area (social work, sociology, psychology, family ecology, consumer/community services, family studies, family and/or child development, guidance/school counseling, counseling psychology, or criminal justice) required along with experience in child welfare or a professional human services environment.
Demonstrated experience working in a human services agency, youth, or community-based organization. Must have experience with case management and child welfare working with youth having aged out of foster care, independent and/or transitional living between the ages of 18-23. Experience should demonstrate use of most of the skills required for this position and in carrying out the kinds of tasks and duties assigned to this position.
Licenses or Certificates:
Valid unrestricted Michigan Driver’s License and provide proof of vehicle registration and auto insurance required. Social work licensure/certification is highly desired.
