GENERAL DUTIES:

The Bilingual Case Manager-Adult Outpatient, is responsible for providing case management services and supports to an assigned caseload of adult clients receiving behavioral health services.  The Bilingual Case Manager is primarily responsible for assisting clients with case management and care coordination needs that are goal-oriented and individualized. The Bilingual Case Manager is responsible for the development of an integrated treatment plan in collaboration with the client and other service providers. Additional responsibilities include linking/coordination of services, reassessment, follow up, advocacy and monitoring of services. 

This position primarily provides home-based, outreach and community-based services. The Bilingual Case Manager is responsible to the Clinical Manager.

QUALIFICATIONS:

Fluency in Spanish Required

Possession of a  Bachelor’s Degree in Social Work or Master’s Degree in Social Work, Psychology or Counseling from an accredited school.

One to three years of case management experience.

Previous experience working with adults with psychiatric disorders.

Appropriate state licensure required (LLBSW, LBSW, LLMSW, LMSW).

Possession of a valid driver’s license and access to a private vehicle for day-to-day job performance.

SALARY:

37,960-49,778 with benefits (Full-Time/Union) w/benefits
