About the Opportunity:
Ypsilanti Meals On Wheels is committed to the belief that all seniors have the right to live
independently, with dignity and respect, for as long as is safely possible.Ypsilanti Meals on Wheels
(YMOW) is seeking a Social Worker to improve the in-home safety of older adults and medically
fragile adults who reside within the YMOW service area, helping them to continue living safely and
comfortably at home.


Position Summary:
This position will work to help meet the needs of older adults and medically fragile adults by
completing medical biopsycho-social assessments, training and supervising students, and case
management support for unmet needs via our resource advocacy program. Additionally, this position
will provide social work support to YMOW’s Community Aging-In-Place Advancing Better Living for
Elders (CAPABLE) Program and Care on Wheels programs. The CAPABLE program is an evidence
based client driven program that has an interdisciplinary team of occupational therapists, nurses,
social workers, and a handyman. The social worker assists with screening eligible clients, identifying
areas of crisis, and using motivational interviewing skills to help clients identify goals and develop
action plans.


Primary Duties and Responsibilities:

  • Conduct biopsychosocial health assessments to identify program eligibility and unmet needs
  • Manage assessments, including keeping track of when assessments are due, assigning students to complete assessments, and reviewing student assessments for accuracy
  • Use case management skills to help homebound clients meet unmet needs
  • Assist with supervising YMOW student interns
  • Provide support and guidance to Community Health Worker
  • Maintain positive, effective communication skills in all interactions
  • Act as a liaison between clinical team, clients, and students
  • Adhere to YMOW organizational policies and procedures, including HIPAA standards
  • Maintain active and current registration and/or licensure via LARA
  • Assist with grant writing for project funding
  • Assist with coordination of program services
  • Other duties as assigned

Required Qualifications:

  • Masters of social work degree from an accredited program with valid LLMSW license
  • Experience working with older adults and/or medically complex adults
  • Ability to work with a collaborative team, strong attention to detail, and good communication skills
  • Reliable transportation with valid driver’s license in good standing
  • Must be comfortable working with diverse clients in their homes (remote/in-person hybrid currently due to the pandemic).

Preferred Qualifications:

  • Experience working on an interdisciplinary team
  • Familiarity with Washtenaw County resources
  • A Master’s degree in social work with a concentration in Gerontology
  • M-MAP certification

Full-time work will be considered for this position. Compensation is commensurate with experience.
Employee benefits for full time employees, covered completely by YMOW, include: base medical and

available. Paid vacation and sick time. Additional benefits include a 401k with up to 3% matching,
and a cell phone stipend.


Salary range starting at: $46,000
