EVALUATION AND ADMISSIONS SPECIALIST I &II

DEPARTMENT:  Access

REPORTS TO: Program Supervisor

Summary:

The primary purpose of this position is responsible to provide triage, crisis service, initial assessments, pre-screen assessments, level of care authorizations for persons requesting service of the Monroe Community Mental Health Authority (MCMHA.)

Responds to initial requests for services on a walk-in, and phone-in, and scheduled basis. In these initial contacts emergency services are provided on an as‑needed basis. 

Based on professional assessment:

  1. initial psychosocial assessments will be completed by the assigned program staff; 
  2. may refer cases to other community agencies consistent with interagency referral agreements;
  3. provide face to face biopsychosocial assessments and make referrals to community resources.
  4. may open cases  to the Access Department with an Emergency Services status as clinically appropriate Responsible for authorization of initial/interim treatment plan and services; 
  5. assignment of case to Clinical Departments of the agency (consistent with program assignment criteria and clinical protocols for service intensity Continuous Stay and authorization for acute service, inpatient or outpatient services shall occur as requested.)  

May be assigned monitor­ing responsibilities at community care taker sites (such as the jail, group homes, and integrated healthcare settings or nursing home) and/or be assigned training or program consultation duties working in collaboration with diverse populations and community agencies. 


Duties and Competencies Assessed by Performance Evaluation(s): 

  1. Demonstrates ability to identify severity of illness by assessing symptom acuity, functional abilities, and risk factors.
  2. Demonstrates ability to accurately assess psychiatric symptomatology and risk of harm to self or others.
  3. Demonstrates ability to assess substance abuse/dependence.
  4. Demonstrates ability to complete an accurate 5-Axis diagnosis based on Diagnostic and Statistical Manual (DSM) and knowledge of corresponding ICD codes.
  5. Demonstrates ability to develop an evaluation of what is causing a crisis situation and develop a plan of safety, which includes alternative resources to hospitalization.
  6. Demonstrates ability to perform pre-screening for admissions for psychiatric hospitalization.
  7. Demonstrates knowledge of needs of people with developmental disability, mental illness, and children with severe emotional disturbance.
  8. Demonstrates knowledge of Duty to Warn, Child Abuse and Neglect, and Elder Abuse reporting laws.

Core Intake/Assessment:

  1. Conducts face to face biopsychosocial assessments with individuals and families as assigned.
  2. Demonstrates ability to develop a biopsychosocial report and determine the appropriate band of care and makes referrals to appropriate departments/programs.
  3. May assess the need for re-opening of closed cases and reassignment.
  4. Screens all incoming calls, providing crisis service for adults, children, adolescents and families, provides face-to-face assessments consistent with authority standards, schedules admission Duties and Competencies KPI PI appointments, or makes referrals to outside agencies or service providers consistent with signed contracts and/or inter-agency service agreements, including but not limited to, Peace Officers, Jail Administrations and staff, Prosecutor Office, Defense Attorney’s Judges and Probation Officers.
  5. Performs phone, walk-in, and/or scheduled assessments.
  6. Assesses the need for re-opening of closed cases and reassignment. Provides re-assessments for second opinions within the Access Department with direct supervision by the Access supervisor.
  7. Coordinates supports/services with the consumer’s identified primary health care provider and upon consent, sends letter to the primary care physician according to agency standards.
  8. Assesses need for psychiatric consultation authorization.
  9. Provides individual and group community education by phone and/or by group presentations.
  10. Works in conjunction with other Access System staff to triage, coordinate the provision of appropriate crisis intervention, emergency service status program placements, follow-up, and after care, and to provide initial and continued stay level of care authorizations.
  11. Complete initial assessments and all related documentation within the 14-day agency standard.
  12. Assigns consumers to appropriate Clinical Department of the authority based upon triage assessment and provides level of care authorizations for continuing care consistent with authority standards.
  13. Maintains and updates a master community resource directory for supporting and enhance consumer functioning and service referrals. Provides information and prepares periodic reports in an accurate manner and a timely basis.
  14. Maintaining complete and detailed case records and service activity documentation consistent with DCH best practice guidelines and standards, authority procedures and requirements Duties and Competencies KPI PI of third party payer sources.
  15. Implements interdisciplinary peer review recommendations and decisions on a retrospective and prospective basis.

Core Emergency Services/Tasks: 

  1. Completes assessments/pre-screens, determining appropriateness of emergency psychiatric hospitalization to State Psychiatric facilities and community hospitals.
  2. Responds to consumer requests for emergency services via phone or face-to-face contact consistent with authority standards in an effort to stabilize the crisis situation.
  3. Provides safety assessments for County schools within the agency standard of two business days.
  4. Completes assessments of children and adults to determine appropriateness of emergency psychiatric hospitalization. Completes assessments for inpatient admission to Walter Reuther Hospital, Hawthorn Center, or private hospital. Completes alternative emergency service status placements when assessments determine they are appropriate.

Performs other related duties as assigned: 

  1. Transportation of consumers when necessary.
  2. Maintains individual credentials and participates in education and training necessary to maintain current competency(s) in skill areas essential for implementing job duties and responsibilities.
  3. Follows oral and written direction completing tasks as assigned by due dates.
  4. Maintains working knowledge of software applications used in performance of daily duties. 

Corporate Compliance Responsibilities: 

  1. Complies with and maintains competence with all applicable laws, regulations, standards, and rules governed under Medicare, Medicaid, the Michigan Department of Community Health (MDCH), Michigan Department of Human Services (MDHS), Joint Commission, and any applicable funding sources.
  2. Understands, adheres and maintains competence with all MCMHA and CMHPSM standards, policies, procedures as they appear in MCMHA’s, Management of Information, Finance-Contract Management, Recipient Rights, Performance Improvement, Administration, Human Resources, Treatment Foster Care, Environment and Provision of Care Policies.
  3. Complies with and maintains competence with the Health Insurance Portability and Accountability Act (HIPAA) regulations as they pertain to privacy and confidentiality.
  4. Interacts appropriately with Managers and peers. Performs required duties with an awareness of impact upon the Authority structure as a whole.
  5. Works as a Team Player within the entire Authority Structure.

JOB REQUIREMENTS:

Education, Training and Licensing Credentials:

Position 1:  Masters Degree in Social Work or Counseling with current licensure. 

Experience:  Two years previous experience with Mentally Ill or Co-Occurring (MI/Sub­stance Abuse) populations required.   One year experience with children with SED and adults/children with a developmental disability required.  Must be able to type at a rate of 35 wpm to complete all assigned paperwork daily.

Position 2:  Masters degree in Psychology with current licensure due to administering psychological testing. 

Experience:  Two years experience in diagnosis testing and two years previous experience with Mentally Ill/Co-Occurring (MI/Sub­stance Abuse) populations required.   One year experience with children with SED and adults/children with a developmental disability required.  Must be able to type at a rate of 35 wpm to complete all assigned paperwork daily.


Other Competencies and Abilities:

The following physical and mental abilities are required:  ability to work in a complex, detail oriented, deadline and structured environment;  frequent lifting or carrying up to 15 pounds, must be able to hear, frequent standing/walking, ability to communicate effectively in person and/or phone, the ability to read, analyze and interpret information, ability to write correspondence and procedures, ability to maintain records,  ability to define and  solve practical problems, collect data, establish facts and draw valid conclusions, ability to influence others. Demonstrated proficiency in Microsoft Outlook, Word and such other applications if/when required.  Ability to be trained and successfully complete other computer training as required. Valid driver’s license, proof of current automobile insurance and driving record determined by the Authority’s insurance carrier to be eligible for coverage if required as part of positions duties.
