Home Based Services Clinician

Summary:

 Mental Health home based service programs are designed to provide intensive services to children with Serious Emotional Disturbance (SED) and families with multiple service needs who require access to a continuum of mental health and other services.  The primary goals of these programs are to promote normal development, promote healthy family functioning, support and preserve families, reunite families who have been separated, and reduce the usage of, or shorten the length of stay in, psychiatric hospitals and other substitute care settings.  The family unit is the focus of treatment.  The service style must support and empower families, parent and professional teamwork and community collaboration with other service providers.

The Home Based Services Clinician will be responsible to perform psychosocial assessments and develop Family/Person Centered Plans.  Services relating to plan goals will be provided primarily in the family home or a setting of the family’s choosing.

Mental Health home based services programs combine individual therapy, family therapy, crisis intervention, case management and family collateral contacts.  Services provided in a home based program vary from assisting consumers in meeting basic needs such as food, housing and medical care to more therapeutic interventions such as individual or family therapy.  Services begin with an assessment of the family’s strengths.

Referrals are received either from the Authority's Access Department or other Agency programs, via Access.   Typically, the length of treatment will be less than a year. Two or three contacts per week per family is the service level more likely to occur because of family need.

The Home Based Services Clinician may be required to provide crisis support services, possibly to include pre-screening for inpatient or partial hospitalization, if needed for assigned consumers and families.

Assesses the effects of the family on the condition of the child. 

  • Assesses the effects of the child upon the family.
  • Assesses all members of the family unit.
  • Understands and appropriately use knowledge of basic growth and development and learning issues of children.
  • Assesses and teaches parenting skills.
  • Assesses functioning in areas of school, community, family and interpersonal relationships.
  • Assess growth and development of the child.
  • Understands and appropriately uses strengths of the child and family.
  • Understands and appropriately uses natural supports within and outside of the family unit.
  • Development of a Family/Person Centered Plan (FCP/PCP) for registered consumers of the Authority assigned to the Home Based Services Program. ·
    • Complete Psychosocial Assessment and all assessments/evaluations necessary to develop FCP/PCP recommendations according to procedures and formats required by the Authority and its funding sources.
    • Develop FCP/PCP goals and objectives appropriate for the consumer’s needs promoting maximal functioning in the community.
  • Implementation of the FCP/PCP for each Home Based services consumer. This includes:
    • Provide follow-up and outreach services directed at achieving FCP/PCP goals and objectives. These services are community based, thus services in family homes, schools and other community settings will be in the majority.
    • Monitor outcome of efforts directed toward service plan goals and objectives.
    • Consult with the program psychiatrist around medication concerns and treatment plans.
    • To assist family members to accomplish their goals based on family’s strengths.
    • To provide collaborative services to CMH staff persons or other agency personnel in order to enhance consumer functioning.
  • Implementation of the FCP/PCP for each Home Based services consumer. This includes:
    • Provide follow-up and outreach services directed at achieving FCP/PCP goals and objectives. These services are community based, thus services in family homes, schools and other community settings will be in the majority.
    • Monitor outcome of efforts directed toward service plan goals and objectives.
    • Consult with the program psychiatrist around medication concerns and treatment plans.
    • To assist family members to accomplish their goals based on family’s strengths.
    • To provide collaborative services to CMH staff persons or other agency personnel in order to enhance consumer functioning.
    • Coordination of referral and services with other community agencies.
    • Complete periodic FCP/PCP reviews with the consumer and family, along with required assessments, status reports and progress notes.
    • Hold FCP/PCP team meetings if reviews indicate that a significant revision of the FCP/PCP is warranted to continue service delivery.
    • Participate in interagency planning and service provision meetings, to assist in providing seamless services.
    • To provide individual, couple and/or family therapy to assigned consumers consistent with the FCP/PCP, clinical diagnosis of the consumer and requirements of third party funding sources.
  • Provide transportation as needed in order to assist consumers and families in procurement and maintaining of residential, daily living and supportive services; linking financial and medical benefits, legal and habilitation services.
  • Maintain an up-to-date master case load list and reviews periodically with supervisor.
  • Maintain complete and detailed case records and service activity documentation consistent with DCH standards, Authority procedures and requirements of third party payor sources.

Perform other related duties as assigned:

  • Maintain individual credentials and participate in education and training as necessary to maintain current competency(s) in skill areas essential for implementing job duties and responsibilities. Minimal annual training hours is 24, with all of these specific to consumer population served.
  • Makes presentations to the public including school, civic and community agencies and relevant meetings, as needed.
  • Maintain current skills and developing new skills to maintain current competence essential for implementing job duties and responsibilities.
  • Follows oral and written direction completing tasks as assigned by due dates.

Corporate Compliance Responsibilities:

  • Complies with and maintains competence with all applicable laws, regulations, standards, and rules governed under Medicare, Medicaid, the Michigan Department of Community Health (MDCH), Michigan Department of Human Services (MDHS), Joint Commission, and any applicable funding sources.
  • Understands, adheres and maintains competence with all MCMHA and CMHPSM standards, policies, procedures as they appear in MCMHA’s, Management of Information, Finance-Contract Management, Recipient Rights, Performance Improvement, Administration, Human Resources, Treatment Foster Care, Environment and Provision of Care Policies.
  • Complies with and maintains competence with the Health Insurance Portability and Accountability Act (HIPAA) regulations as they pertain to privacy and confidentiality.
  • Interacts appropriately with Managers and peers. Performs required duties with an awareness of impact upon the Authority structure as a whole.
  • Works as a Team Player within the entire Authority Structure.

  • Complies with State DCH and MCMHA Board policies, standards and procedures related to Recipient Rights.
  • Maintain competence in HIPAA and other regulatory requirements.
  • Understands and adheres to MCMHA's compliance standards as they appear in MCMHA’s, Management of Information, Recipient Rights, Performance Improvement, Administration, Human Resources, Environment and Provision of Care Policies.
  • Interacts appropriately with Managers and peers. Performs required duties with an awareness of impact upon the Authority structure as a whole.
  • Works as a Team Player within the entire Authority Structure.
  • Keeps abreast of all pertinent federal, state and MCMHA regulations, laws, and policies as they presently exist and as they change or are modified.

JOB REQUIREMENTS:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed must be representative of the knowledge, skills, minimum education, training, licensure, experience, and/or ability required. Education, Training and Licensing Credentials:

Masters degree in social work, psychology or related field with corresponding Michigan licensure. Must be trained and have one year of experience in the examination, evaluation and treatment of minors and their families. Prefer one year of experience with emotionally disturbed youth in home based service delivery, crisis intervention and substance abuse treatment. Must be able to work well with staff from other agencies.

Demonstrated knowledge and skills necessary to provide the appropriate care for the specific clinical needs and ages of the individuals served. Demonstrated knowledge of community and family systems, symptoms, and behaviors of individuals served, as well as understand the principles of growth and development over the life span; have the ability to assess data reflecting each individual’s status; interpret the appropriate information needed to identify each individual’s requirements in relation to his or her age-specific/population specific needs; and to provide the care needed by the appropriate age group - (child, adolescent and adult). This is typically done in concert with staff and individuals from other community agencies.

Other Competencies and Abilities:

Ability to work in a complex, detail-oriented, deadline structured busy organization. The following physical and mental abilities are required: frequent lifting or carrying up to 15 pounds, must be able to hear, frequent standing/walking going up and down stairs due to community outreach basis, ability to communicate effectively in person and/or phone, the ability to read, analyze and interpret information, ability to write reports, correspondence and procedures, ability to maintain consumer charts/records, ability to define and solve practical problems, collect data, establish facts and draw valid conclusions, ability to influence others, and the ability to conduct patient assessments and treatment. Demonstrated proficiency in MS Outlook, MS Office, electronic medical records (Encompass II) and such other applications if/when required. Ability to be trained and successfully complete other computer training as required. Valid driver’s license, proof of current automobile insurance and driving record determined by the Authority’s insurance carrier to be eligible for coverage if required as part of positions duties.
