Common Ground's core purpose is helping people move from crisis to hope, which is carried out through caring and empathetic staff who work in crisis programs across the crisis continuum. As part of the Oakland Crisis Intervention Recovery Team, which is a mobile crisis team, the OCIRT Crisis Clinician is a team partner with a Recovery Coach who is a Certified Peer Support Specialist. Together much of the time, they respond on-site to crisis calls that cannot be resolved by the Resource & Crisis Helpline telephonically/text/chat in Oakland County. In addition, the team responds to locations as directed by the Crisis Assessment Team Shift Supervisor.

SUMMARY:

The Crisis Clinician utilizes best practices in crisis intervention in behavioral health (mental health and substance use) prevention and post-intervention crisis services, education, crisis stabilization services, short term treatment (i.e. strength-based crisis therapy, cognitive-behavioral interventions, mindfulness approaches, anger management, and coping skills), and follow up services until connected to an ongoing services provider. The team serves children, adolescents, adults, senior adults and families as well as schools, neighborhoods, organizations and businesses by providing up to 30 days of in-home crisis counseling to people in crisis to facilitate immediate stabilization of emotions and behavior. OCIRT is supported by the resources of a crisis center staff which includes psychiatrist and nurses, as well as a continuum of crisis services for which the Crisis Clinician has access.

$3000 to clinicians over 6 months: at hire, 90 days and at 6 months

SUMMARY:

The Crisis Clinician is a part of the Crisis Assessment Team, which is comprised of Welcoming Specialists, Crisis Traffic Coordinators, Oakland Crisis Intervention Recovery Team (OCIRT) Clinician and Recovery Coach, and the Shift Supervisor. The Crisis Clinician role provides 24-hour on-site and mobile assessment that focus on meeting the needs of all populations in crisis, emphasizing alternatives to hospitalization and continued support. The Crisis Clinician is an at-will position that reports to the Shift Supervisor of the Crisis Assessment Team. Clinicians in this role work with individuals and families/caregivers who are experiencing behavioral health/ substance use crises in the hospitals, or on site as walk-ins or via police and/or ambulance using the emergency entrance.

ESSENTIAL JOB FUNCTIONS:

Provide triages and assessments in hospitals and at the Resource & Crisis Center that include collateral contacts with Core Provider Agency and other people who know the individual well and who are involve in his/her life; crisis interventions and assessments are trauma informed and recovery-oriented

Assess crisis and person/family's needs to determine service needed

Document all required and collected information in the Electronic Health Record (EHR) using Collaborative Documentation with persons served

Develop the plan of care in collaboration with the individual/family and the Core Provider Agency based on strengths, needs, abilities, and consideration of preferences, and provide individuals served with due process/second opinion, etc.

Ensure that eligible individuals who are released or diverted from jail are successfully linked to the most appropriate level of care

Act as an advocate for the needs and rights of every person/family including service providers and community agents

Complete mobile emergency assessment and recommend level of care for private/public insurance contracted with Common Ground

Make referrals and provide authorization for services

Participate in daily team huddles

Work closely with other Common Ground programs, crisis safety-net providers, Oakland Community Health Network, core provider network, area emergency departments, and the access system

Comply with attendance and punctuality policies, as well as all other Common Ground policies and procedures

SUMMARY:

The Crisis Interventionist II (CI II) is an at-will position that will report to the Genesee CIRT Program Manager. The CI II will provide crisis intervention and support to individuals/families who are experiencing a mental health, substance abuse, and/or family crisis. This position will work closely with crisis safety-net providers, CMH/provider network, area emergency departments, and the access system. The CI II will assist people/families in accessing community or CMH resources and services. This position will promote and participate in activities that enhance recovery and family resiliency.

ESSENTIAL JOB FUNCTIONS:

Briefly assess crisis situation and person/family's needs

Actively engage person/family experiencing a crisis in order to avoid negative outcomes

Provide support and assist people/families in building trust and rapport

Utilize recovery principles while comforting people in crisis: Hope, Choice, Empowerment, Recovery environment, Meaning and Purpose

Develop understanding and management of wellness and triggers

Provide active crisis intervention that is welcoming, trauma-informed, brief, and solution-focused to adults/children presenting with emergent or urgent need

Join with person/family to create a comprehensive crisis plan and safety plan following Person/Family Center Planning principles that utilize the individual's strengths, natural supports and community resources, and that supports crisis mitigation in the least restrictive environment

Arrange or provide transportation to a safe place as needed

Educate and assist significant others and family members with the petitioning process

Encourage consumers to be actively involved with planning and treatment

Explore and link person/family with natural and community supports

Provide referrals, arrange for psychiatric evaluations, facilitate authorization for hospitalization if unable to successfully divert, and inter- and intra-agency services

Facilitate a respite plan for individuals who are homeless or who do not feel safe in their home environment

Facilitate transition to appropriate aftercare services and assist with implementation of individual recovery plans and coordination of natural supports

Collaborate with other providers, the criminal justice system, including CMH, Mental Health Court, and Adult Drug Court staff to ensure that eligible individuals who are released or diverted from jail are successfully linked to the most appropriate level of care

Act as an advocate for the needs and rights of every person/family including with service providers and community agents

Comply with federal, state, county, and agency regulations governing Targeted Case Management and the delivery of specialty services and supports

Provide first aid and emergency procedures as needed

Submit all electronic or hard copy documents are complete, accurate, and signed prior to end of shift

Complete Service Activity Logs in accordance with departmental standards

Adhere to established Quality Improvement Indicators

Comply with attendance and punctuality policies, as well as all other Common Ground policies and procedures
