The candidate for this job will develop and complete participant’s care plan; establishes goals and directives of the care plans, and services to be utilized. Maintains regular, ongoing contact with participants, caregivers, and physicians. Conducts reassessments and monitors participant progress. Completes and submits timely paperwork and reports.
Essential Duties and Responsibilities Strategy
Utilizing assessment, develops a plan of care directed by the participant (Person Centered Care Planning) • Performs assessments on Person Centered Planning meeting (PCPs) on designated assigned cases. • Monitors assessments and Person-Centered Planning (PCPs) • Develops and completes care plans and other required documents. Reviews and modifies care plans as directed by the participant / caregiver during the assessment Person Centered Planning and obtains the participant’s signature of approval. • Maintains regular, ongoing contact with participants, caregivers, physicians, and service providers listed on service plan through home visits and phone calls. • Educates participants, caregivers and service providers regarding the roles and responsibilities of each party, assuring that services are provided as documented in the care plan. • Conducts Nursing Facility Level of Care as required. • Continues to evaluate the appropriateness of the service plan, adjusting it as needed. • Monitors participant progress in relation to goals and objectives listed in the care plan. • Interfaces with community resources as needed to support participant. • Collaborates with peers (RN or SW) in coordinating services. • Produces timely and accurate commuter entry reports • Maintains participant confidentiality • Participates in training, supervisory meetings, in-services, and continuing education opportunities. • Participates in other duties as assigned. Education:
Bachelor’s degree in Human Services or related field, preferred, equivalent experience acceptable. Current Michigan driver’s license / insurance and access to a vehicle required
Experience:
• Two-three years of experience completing intakes, pre-screening, and/or assessments, preferably in a health care or human service setting serving the elderly and/or disabled population. • Knowledge of Medicaid and Medicare regulations. • Ability to complete assessments using computerized screening tools and/or MS office • Excellent communication skills, written and verbal • Excellent Customer Service skills and ability to solve problems • Home care / Field experience.
