CLASSIFICATION TITLE: Intensive School Based Therapist - Contractual

DEPARTMENT: Community Mental Health

F.L.S.A. STATUS: Non-Exempt

APPOINTING AUTHORITY: Executive Director

GENERAL RESPONSIBILITIES:

Under direction, provides assessments, linkage and referrals to children in enrolled in Macomb County schools.  Maintains direct and indirect clinical and administrative records, coordinates, consults, and provides liaison work with other agencies and system. Therapist will also be a member of the school team and will assist with the development and implementation of an appropriate coordinated plan of care in order to provide behavioral health support and services to general education children.

ESSENTIAL FUNCTIONS:

Performs direct clinical services, including, but not limited to, case management, therapy, and crisis intervention.

Completes necessary documentation such as, assessments, treatment plans, transition plans, and crisis plans.  Collects and provides necessary data to MISD coordinator.

Works as a team member within the school and provided support and training to school personal as needed.

Works with the child/family to obtain needed resources within the community and makes all necessary referrals. 

Participates in agency and system wide staff meetings and mandatory trainings. Participates in supervision as scheduled.

Utilizes computers to perform clinical and administrative job functions.

EDUCATION, TRAINING AND EXPERIENCE:

Master’s Degree in Social Work, Counseling or a directly related Mental Health field from an accredited college or university, with appropriate licensure.

Must meet qualifications of QMHP.

Possession of required state licensure appropriate to position and ability to meet privileging and credentialing guidelines.

Successful completion of the probationary period for the position of Intensive School-Based Therapist.

Be physically able to perform the essential functions of the position, with or without reasonable accommodation.

Experience with trauma, CBT, and other evidenced-based practices are preferred.

ADDITIONAL QUALIFICATIONS:

Working knowledge of current information of appropriate resources within MCCMH and outside MCCMH.

Considerable knowledge of the Michigan Mental Health Code.

Ability to make independent decisions when circumstances warrant action.

Ability to act as both a member of a team and/or a team leader.

Ability to work effectively with minimal direct supervision.

Effective written and oral communication skills.

Ability to establish and maintain effective working relationships with consumers, staff, health professionals, physicians, consultants, and governmental agencies.

Ability to maintain the confidentiality of the consumer and family.

Possession of a valid Michigan driver’s license and an operable insured automobile for authorized travel.

Ability to conduct oneself with tact and courtesy.

JOB SUMMARY:

Under direction, provides home based therapy, conducts screenings, performs intake assessments and reviews case management services within a Person Centered Planning approach to infants and their parents. Requests referrals for ongoing services within Macomb County Community Mental Health (MCCMH) direct operated or contract programs with the goal of moving consumers to a level of care consistent with their clinical presentation. Maintains clinical and administrative records; coordinates, consults and provides liaison work with other agencies and systems; ensures service authorizations are current; implements program goals and activities in keeping with program philosophy. Performs related duties as assigned.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

Performs direct clinical services, including, evidenced based therapy, assessments for infants and toddlers, and their families, crisis intervention; intakes, treatment plans; case management and transition services.

Performs indirect clinical services, including, timely and complete case documentation following MCCMH policies and procedures; confirms diagnosis to make the most suitable, least restrictive and medically necessary clinical recommendations; and assists client and/or family in obtaining appropriate programs and resources.

Provides follow up case management services for consumers to assist in obtaining necessary services, coordinating those services until the consumer is transitioned to an ongoing service provider.

Maintains clinical documentation accurate and up-to-date, including, assessments, progress notes, treatment plans and other required documentation.

Ensures administrative paperwork is accurate and up to date, including, staff activity logs, statistical reports, attendance, mileage reports, and case lists.

Participates in agency and system staff meetings.

Conducts thorough, accurate, and clinically sound triage/screening with consumer/guardian/family members in a timely manner.

Requests appropriate authorizations for services provided; monitors utilization of services; requests re-authorizations or continuations as appropriate.

Serves on community mental health or community based committees as directed.

May supervise the work of department interns.

May assume the administrative responsibilities of a Therapist III in their absence. Obtain and maintain a Michigan driver's license prior to time of hire.

KNOWLEDGE, SKILLS AND ABILITIES:

QUALIFICATIONS (Education and Experience):

REQUIRED:

Master's Degree in Social Work, Counseling, Psychology or Doctorate in Psychology or a directly related mental health field from an accredited college or university.

Licensed Master Social Worker (LMSW), Limited Licensed Master of Social Work (LLMSW), Licensed Professional Counselor (LPC), Limited License Professional Counselor (LLPC), Temporary Limited Licensed Psychologist (TLLP), or Licensed Psychologist (LP).

Possession of required state licensure appropriate to position and ability to meet privileging and credentialing guidelines.

Three (3) years clinical experience working with infants and toddlers.

IMH level 2 endorsement, or state waiver in place

CONTRACT POSITION DESCRIPTION

Classification Title:              Therapist – Children’s – Wraparound Facilitator

JOB SUMMARY:

Under direction, provides assessments, linkage and referrals to consumers seeking Macomb County Community Mental Health services, maintains direct and indirect clinical and administrative records, coordinates, consults, and provides liaison work with other agencies and systems, ensures that service authorizations are current, implements program goals and activities in keeping with program philosophy, contributes expertise from professional discipline to team planning and treatment processes; performs related duties as assigned.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

Performs direct clinical services, including, but not limited to, intakes, treatment plans, services reviews, crisis/safety plans, and individual contacts.

Provides hands on family training, if needed, to all assigned consumers in the wraparound program.

Consults with school personnel, juvenile court, CPS, Foster Care workers, as well as community based connections in order to provide the optimal level of support and integration.

Functions as a member of the wraparound treatment team, and meets weekly and more if necessary with the family and other team members in order to assist in facilitating the family wraparound plan and transition/discharge planning.

Performs indirect clinical services, including, but not limited to, timely and complete case documentation following Macomb County Community Mental Health policies and procedures; participates in case consultation to confirm diagnosis and to make the most suitable least restrictive clinical recommendations.

Participates in agency and system staff meetings. Participates in weekly supervision.

Serves on Community Mental Health or community based committees as directed.

Completes wraparound 3 day new facilitator training given by DCH.

Completes 2 wraparound trainings per calendar year to maintain certification.

Functions as a team leader and facilitator working with a family to develop a team, identify outcomes, underlying needs, strengths, and action steps to reach goals.

Facilitates monthly team meetings with the family, consumer, and team.

Utilizes computers to perform clinical and administrative job functions.

Follows wraparound best practice values.

Successful completion of the probationary period for the position of Therapist II.

PREFERRED:

One (1) year or more of experience working in a community mental health setting. 

Completion of Three Day New Wraparound Facilitator Training.

One (1) year of more of experience working as a wraparound facilitator.  

Four plus years previous experience in the assessment and treatment of children, including, but not limited to juvenile justice population, SED children, and IDD children.

SKILLS AND SPECIFICATIONS:

WORKING CONDITIONS/PHYSICAL DEMANDS:

Works in an office environment, as well as in the community, and uses a computer, telephone and other office equipment as needed, to perform duties. The noise level in the work environment is typical of that of an office with frequent contact with customers.

Bends, stoops and reaches in order to file, search for and retrieve records and documents.

Manual dexterity and regular fine-finger and hand/wrist motions are required for operating a keyboard, writing and filing.

Seeing (vision), sitting, talking/speaking, or listening/hearing are regularly required.

Operates an automobile while performing assigned job duties.
