What you would be doing: 

As a member of the interdisciplinary team, the Social Worker provides services to respond to the emotional needs of residents and their families from admission to discharge. You will conduct assessments, assist with admissions, transfer discharge and other paperwork, provide, or refer to proper support services, monitor and report changes to team members and family as appropriate.   


What your work schedule would be: 

 This position works regular business hours Monday through Friday.  


The successful candidate: 

  • Has a minimum of Bachelor’s Degree in Social Work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology, Master’s Degree preferred.   

  • Has a minimum of one year supervised social work experience working directly with individuals in a long-term or sub-acute care rehabilitation. 

  • A preference for a current Social Work license in the state. 

  • Current knowledge of state and federal laws and regulations that apply to practice of social work in long-term care.
